How long will my report be available here?
This will depend on the data processing arrangements in place with your health check provider. It is a good idea to use My Well Record® to save a permanent PDF copy of your report.
Who do I contact if I have questions relating to my health check results?
Please contact your health check provider directly, or discuss this with your GP. If the provide that gave you your health check was set up to transfer health check results to GP practices, your GP should have received a copy of your report.
How many times can I request a one-time code?
There is no limit to the number of times you can request a one time code but it will only be needed once per device/browser.
Who should I contact if I have a query about the results of my recent health check?
please contact your health check provider directly. if your results were transferred to your gp, they will also received a copy of your report. if this is the case, you can also discuss your results with them.
Will my GP have access to this information?
Depending on the organisation that performed your health check, your GP might have received a a full report of your health check. You should have been advised of this at your health check.
What happens when a unique key is entered incorrectly?
If you don't use the correct unique key to confirm your identity, you won't be able to access your report. If you need the unique key again, contact your health check provider.
Can I choose to receive my one-time code as a text or notification on my mobile phone rather than by email?
Only email validation can be used to confirm your identity and to ensure your My Well Record® is private to you.
How long will my one-time code be valid for?
One-time codes are valid for 15 minutes after they are sent to your email. You can always request a new one if this time runs out.
How do I delete my report from this website?
Your right to be forgotten is important, so if you wish to have your report and details removed then please contact your health check provider directly with your request.
The links in my health report don't go anywhere, what do I do?
Your health check provider is responsible for the availability of the services available via the links. Please contact your health check provider directly if the links are no longer valid.
